This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. We are required by law to maintain the privacy of your protected health information (PHI) and to provide you with this Notice.
How we may use and disclose PHI
We use and disclose PHI for treatment (coordinating your eye care with other providers), payment (billing your insurance), and healthcare operations (quality improvement, audit, training). We may also use PHI for appointment reminders, treatment alternatives, and as required by law.
Disclosures requiring your authorization
Most uses or disclosures of PHI for marketing, sale of PHI, or psychotherapy notes require your written authorization. You may revoke an authorization at any time.
Your rights
You have the right to: request to inspect and copy your PHI; request an amendment of your PHI; receive an accounting of disclosures; request restrictions on use and disclosure; receive confidential communications by alternative means; and receive a paper copy of this Notice.
Complaints
You may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.
